Clinical Analysis of Colorectal Tumor, 200 cases


Abstract
The incidence of the colorectal carcinoma reveals lower in Korea than that of the Western Country. It is probably due to fiber rich diet and lower incidence of premalignant lesions, such as polyps and ulcerative colitis.
This study is clinicopathological analysis of 164 patients of colorectal cancer and 36 patients of benign tumor who were diagnosed histologically and subjected to surgical operation at St. Mary¢¥s Hospital, Catholic Medical Center, during the 10 year interval from Jan. 1, 1963 to Dec. 31, 1972.
As the results of this study, the following data were obtained.
1. The incidence of the colorectal carcinoma was high and more frequent than the carcinoma of other organs:
2. The average ages of the patients were 50.4 year in carcinoma, 7.7 year in juvenile polyp and 41.5 year in adult polyps. The ratio of the colorectal¢¥ carcinoma in male and, female was 1.4: 1 and 1.25: 1 in benign tumors.
3. The average duration, from onset of symptoms to hospital visit, was 6.5 months.
4. The carcinoma found mostly on rectum (41.4%) and then sigmoid.
5. In the cases of right, colon carcinoma, there were pain, palpable mass and anemia, and in the left colon cancer, there were pain, bloody stool and constipation. The rectal cancer showed change of bowel habit. In the cases of benign tumors, the rectal bleeding was the most common symptom.
6. The associated diseases of the carcinoma were hemorrhoid (20.1%), fistula and polyp.
7. Resectabiliy of carcinoma was 76.2% (curative resection 48.2% and palliative resection 28.0%).
8. In the carcinoma of the large bowel, the constrictive type (60.6%) was the most common -type, and the polypoid type was mostly on the right colon. In the benign tumors of the color-etum, the adenomatous polyp was the most common type. The metastatic rate was high on constrictive and undifferentiated form of carcinoma.
9. The wound infection was the most common among the postoperative complications (36.6%) of carcinoma.
10. The mortality of carcinoma was 4.5%.
11. The follow-up study was possible only in 43 patients, who had undergone the resection of carcinoma (36.4%). According the Duke¢¥s classification, the 5 year survival rates were 60% on stage A and B, 50% on stage C and 0% on stage D. The overall, 5 year, survival rate was 40¡Æ%0.
